Who played Tuba on the Sinatra / Buddy Rich Concert for the Americas? Forward bell, Glasses?

Was it Don Butterfield?

Good playing, used quite sparingly, but fits well.

That is Don Butterfield. He played with Sinatra for about 4 years.
