I'm wondering if anybody know's where I cam find a form that works for some smaller time work as far as a theater agreeing to be responsible for damages or loss of instruments. All of the tours provide this but smaller theaters don't generally hand out the sheet to fill in, and it seems that there must be a form somewhere. Thanks. ASG
