It is a Romera Brass Mel Culbertson mouthpiece.
They've been discussed here a few times. I'm not sure they are still made or you could still get one, though Romera is still in business with a couple of other tuba mouthpiece choices.

Actually, I think it's a Culbertson Vulcano. The mouthpiece is made by Romera Brass, was made for Mel Culbertson.
I don't know what the "disk" does... by this photo spoils the illusion.

The photo I found was from a Japanese web site. Not sure that will help much, but if you google Romera Mel Culbertson, you'll find some stuff.
