Just curious , does anyone know of a manufacturer who will create a mouthpiece to your own specified dimensions?

Several. Elliot, Kanstul, just to name a couple. I was where you are a few years ago. Instead of going full custom, see if there is someone who makes something similar that only needs a tweak or two. Much simpler. Much cheaper. For example, Jim @ Kanstul modified their 18 from 1.26 to 1.28 cup diameter; everything else was exactly what I was going to spec for a souzy mouthpiece. To have designed it from scratch would have cost double, and would have been no different than the simple modification to the existing offering. Bloke has many rim, cup and shank mix-and-match components, and a spacer to adjust depth. I was able to put together what I needed for my "Bessophone" from existing stock.

These are just a couple of examples. There are many, many mouthpieces out there. I suggest you start with a global chart that lists mouthpieces of your desired cup i.d. and go from there to narrow down the universe of mouthpieces to a few manageable galaxies, and then research different manufacturers. Most now put their specs on line. The most notable exception being Conn, but everybody knows what a 120S and a 7B are, and if you were wanting one of those, you wouldn't be posting this thread.
