Looking for "Elegy for Tuba"

"Elegy for Tuba" was written by John Stevens and printed as GEM No. 6 in the ITEA Journal 32:1 (2004). The on-line version of ITEA Journal displays the article but does not have the music. Does anyone here have the music available and could send it to me?
