www.dillonmusic.com They deal with Cerveny/Amati, and can custom order instruments.

why would you want such a device?!
You will probably start a world tuba war
...and maybe that´s why 5-valved Baritons are REAL scarce even over here.
With 4 valves, these things already cross the border to the F-tuba (and quite annoyingly so, I might add).
These would come in handy with Oberkrainer ensembles, though, ´cause with these, a very large Bariton usually does the bass part to support a clarinet, trumpet, accordion and a guitar.
