What is this?

Today I saw an unusual instrument on German eBay. Looks like some sort of 4 valve instrument, but not the usual style?!

It is from Van Engelen, Lier (french: Lierre)/Antwerpen, Belgium, Lisper Steenweg (fr. Rue de Lisp)
I also have an Van Engelen instrument: A very old 3v Helicon.
