I rebuilt a three-rotor version of one of these cute little horns a while back:
It's indeed Czech... as 'Bloke' suggested. Mine was made by Jerzy Krywalski. The name plate was probably removed from the sellers horn during a repair. (or maybe it fell off as some do!)

I think they are OK to play. I prefer front-action and rotary horns, anyway... so I am more than a little biased.
