It's a B&S F model Symphonie.
By the style of braces, it's older than the 1970s and more likely early 1960s.
Regarding the 5th valve linkage, it was(and still is) possible to order from B&S with either left or right hand/thumb or finger operated including triggers for the first slide etc.
Here's a picture of a "brand new" 1966 5valve version next to a mid 1970s 6valve.

The owner of both horns plays low Bb 1+2+4. He does have the longer 5th valve slides, but prefers the originals.
He also has a longer 3rd valve slide to give a modern 2+3 combination with 3rd alone. It's known as the Dresdener fingering system...
