I'm not sure what their hands look like in India, but I have no clue how anyone could play this.

Or am I missing something here...?

Probably using the left index finger on the 4th valve... like on a 'side-shifter' euphonium.

Ah, well I suppose that explains it. Doesn't look very comfortable, but at least it explains it!
