Does anyone know of a manufacturer who makes a stepped ferrule that measures 21mm on one side and 20mm on the other? (Those measurements are for corresponding OD tubing - the ID would be .787/20mm and .748/19mm, respectively).

If not, is that something that would be practicable on a lathe?

Brass, @ 3/4" long, maybe with a decorative groove at each end, like you'd find on a York?

No, something like a plumbing "reducing coupling"
Probably not really necessary (just expand the smaller tubing to fit a 21mm ferrule).

I know that King tubing ferrules are stepped by design, I thought that there may be others....

Just the opposite, bloke - the outside of the ferrule looks like a normal cylindrical tubing ferrule, but one half is 21mm, and the other is 20mm, no taper.

Similar to how King makes their ferrules.

I suppose I could do that, but it wouldn't be uniform on the outside - it would look sorta like two different sized ferrules brazed together....

would it be bad to use a combination of removing material on the inside of a ferrule and slightly shrinking the end of the larger tube until it fits together well?
