The Kanstul 18 is superior to the Bach 18 in these aspects:
1) consistency - 1.26 Rim I.D. Period. No variance in inside diameter, outside diameter, rim width and contour.
2) tighter throat - has the .328 throat instead of the larger .348 Bach throat - better breath control
3) slightly deeper - less graininess in the low register but no sacrifice to tone, range, response, projection, flexibility and control
If Jim New had not made a custom version for me a few years ago with a 1.28 rim I.D., for the sousaphone I play on occasion, I'd be all over this one.

I don’t have Bach anymore so I can’t answer that. From what I know is that Kanstul copies are more consistent while Bach had some variations. Maybe someone else can also pitch in. But as far as Kanstul and Milton, those measure very close to each other as far as rim, cup/depth. Of course Kanstul has standard/American shank and Milton has Euro shank. From outside look little different, I think Kanstul also slightly heavier. I just posted photo of those next to each other.

I have noticed that there’s a variation between Kanstul 18 mouthpieces. On the mouthpiece where “Kanstul 18” writing is on one side of mouthpiece throat is larger compared to where “18” is stamped on opposite side.
I have one Kanstul 18 that I just posted for sale and throat is smaller for sure. I have round object that’s .32 and it doesn’t go through this Kanstul 18 mouthpiece. I also tried to place same object where Kanstul and 18 stamped on the same side and object falls through with ease.
