Try Stork Custom M P in Vermont, 802-454-1004, they do great work.

I have used two places to have my mouthpieces restored, Oberloh music

Both did a good job for me. Lee Stofer will also do this and although I haven't had a mouthpiece done by him, given my experience with his work I'm sure he would do a great job too.

One place that advertises mouthpiece restoration relatively inexpensively is mouthpiece express, although I don't have any direct experience with their service in this area:
