Cerveny or Amati F would be my guess, but I've never seen a 5 valve model before 8)

My guess is, some old thing with modifications done later. It looks and sounds to me somewhat like the Cerveny F helicon, but with obvious differences. And I don't think it's F. I'm not very good at watching fingers, and it looks far too small, but the fingers seem to be for C?

As for the Hawaii buyer: Either he told the story here on TN or I communicated with him personally, I don't remember which. Point was that there was an attempt to start a community band in his area. I have seen photos from one of their first rehearsals. Very few members, two flutes, helicon, and side drum. Maybe also a saxophone.
