Assuming you've read what Amy said to read and determined that when you're playing etudes you're doing it right, then the only real problem is that you forget what you're doing when you're not playing etudes. Then...the solution really is a concentration one and not a technique one. You have to be very aware of what you're doing when it works and transfer that to situations where it's not working.

I agree with the above postings. I didn't play euph for around three years after high school, so when I arrived at college I had to get back in shape in a hurry and my upper range suffered because I developed some bad embouchure habits by trying to force myself into shape. Eventually, I was able to work out most of my upper range issues. I have found that the key to upper range is to: 1.) Practice CORRECTLY and 2.) Be Patient because you will have a day when the light bulb comes on.
