Looks to me like a Cerveny CBB 883-5, based on the:



- Engraving on bell
Horizontal main tuning slide
Thumb-operated 5th valve
Clockwork springs on 1st through 4th valves
Relative width of 1st and 5th valve slides
Routing of leadpipe
