Can anyone tell me what the horn is this guy is playing? It looks like a handy little horn and the sound is nice.

Yeah, I think these Eb clarinets should be used as 'door stops'.

Looks to me like a Cerveny CBB 883-5, based on the:
  • Engraving on bell
    Horizontal main tuning slide
    Thumb-operated 5th valve
    Clockwork springs on 1st through 4th valves
    Relative width of 1st and 5th valve slides
    Routing of leadpipe
compare screen caps from the video (below) with the picture in the current catalog, page 12, downloadable from: http://cerveny.ecbook.cz/

The guy playing it looks like Tarzan from the Disney Toons.
