Letter of Educational Necessity.

Ok, so I have most of my ducks in a row to take a student loan out to purchase a horn. One of the last things I need is to find a nice sample copy of a formal educational necessity letter that I can adjust to fit my needs, have my instructor sign and take to the financial aid office. Is there anyone who would happen to know where a certain document can be found or what I would need to set up the letter to look professional and of consideration for Financial Aid?
