Need TubaChristmas Registration Form

I'm doing the Ayer, MA, TubaChristmas Sunday, and I can't find my registration forms to copy. I checked the HPF website and didn't find any.

Do any of you fine, upstanding, stouthearted, TC folks have a digital copy I could work from? pdf or jpeg would likely be preferred, but I'll work with anything, even (shudder) bmp!
