Need TubaChristmas Registration Form

I'm doing the Ayer, MA, TubaChristmas Sunday, and I can't find my registration forms to copy. I checked the HPF website and didn't find any.

Do any of you fine, upstanding, stouthearted, TC folks have a digital copy I could work from? pdf or jpeg would likely be preferred, but I'll work with anything, even (shudder) bmp!

Go to www.tubachristmas.com and look in the center of the page, the 6th link down. You can either downlaod a PDF version of the form or you can print the fullscreen version.
