Some places can do rim modifications of an existing mouthpiece.

Contact Jim at Kanstul. They have a lot of mouthpieces in their CNC computer reference. They took one of their stock models and modified the cup and rim for me for a reasonable price, and even sent me a cross-section CNC rendering by email before hand so I could cut out a cardboard "model" to make sure it was what I wanted.

You can easily do what you want with my 3 piece mouthpiece system, and it's not custom.
